When you come for your surgery, please bring the following:
- Driver’s License or a valid ID
- Insurance information, if instructed to do so
- Reports, X-rays, MRIs, CT scans, etc. if instructed to do so
- Complete List of ALL your medications
- Any equipment given to you by your surgeon: crutches, walker, brace, ice machine, etc.
